This includes essential services such as immunizations and screenings, ensuring you and your dependents can access these preventive measures at no cost. TOBACCO-FREE INCENTIVE

Employees and spouses/domestic partners enrolled in a 2026 medical plan can avoid a biweekly tobacco surcharge by completing the annual tobacco-free incentive program.

1. Employees that enrolled during Annual Enrollment in November 2025 have until March 15, 2026 to complete the program.
2. Employees hired after December 1, 2025, have 90 days from their medical plan effective date to complete the program.

IMPORTANT: This program is only for employees & their spouses/domestic partners enrolled in one of our company medical plans in 2026.

To complete the program, employees and their spouses/domestic partners must complete one of the following company-paid options based on timing listed above. To complete the program simply:

1. Register and complete a “negative” Cotinine test* through our lab partner**, or,
2. Register and complete three virtual tobacco coaching sessions through Navigate Wellbeing

Employees and spouses/domestic partners enrolled in medical plan in 2026 that do not complete this program will be subject to a bi-weekly tobacco surcharge deduction beginning the earlier of:

• April 10, 2026, or
• First biweekly period following their 91st day on medical plan.

Employees may complete testing/coaching after the initial deadline to remove the tobacco surcharge on the pay period following the monthly notification from Navigate.

Biweekly Surcharge Amounts:

• Employee only: $34.62 biweekly ($75/month)
• Spouse/Domestic Partner only: $34.62 biweekly ($75/month)
• Employee & Spouse/Domestic Partner: $69.24 biweekly ($150/month)

Navigate Wellbeing will manage the program through wellbeing app/portal. Access instructions for the app/portal and lab registration will be provided to employees and spouses/domestic partners enrolled in a 2026 medical plan via email from Navigate after the medical plan effective date.

A cotinine test is a blood draw test that detects active tobacco or nicotine use (cigarettes, cigars, vaping, etc.) or exposure to nicotine-containing products (e.g., patches, gum, e-cigarettes). Screenings at offsite labs will be completed via a blood draw.
